The virus is still ravaging through Port Charles. Both Mike and Courtney get the virus. Courtney gives birth to a premature baby named John. Courtney dies shortly after childbirth. Both Sonny and Mike are devastated. Mike is angry and lashes out at Sonny.

Sonny: Courtney didn't make it. Courtney's dead.

Mike: Get out.

Sonny: I'm sorry.

Mike: Get out. Courtney meant nothing to you. She was like something on the bottom of your shoe. You couldn't even find it in your own heart to embrace your sister, so don't stand there and act like you're sorry. Don't pretend to grieve with me. And what they say is true. Only the good die young and somehow a selfish bastard like you gets to live forever. Get out of her, Michael. Get out of here! Just leave me alone! Get out of here!

Jason gives Sonny an ultimatum and demands he end things with his sister, Emily. Sonny reluctantly agrees, but warns Jason:

You need to be sure, because your sister is getting hurt. She was raped, her husband abandoned her, and it took a lot of courage for her to trust what she felt with me, emotionally, physically. And now she's going to feel she was wrong, because if I can go back to Carly so easy, you know, then that means that Emily didn't matter, her love had no value. I am destroying Emily's faith in herself.

He lends Mike money for gambling, which he has never done before. His violent temper seems to flair at minor incidents. Emily suspects something is off with Sonny and starts doing some research. After talking with Dr. Lainey Winters, a psychiatrist, about some of Sonny's symptoms, Emily suspects that Sonny might have bipolar disorder. Sonny dismisses Emily's concerns and tells her he is the way he is because of the abuse he suffered as a child.

Sonny: I've had a temper my whole life. I'm sure you've been warned. It gets worse when I get hit or punched because then I really lose it. I get out of control, sometimes I don't even know what I'm doing.

Emily: Doesn't that worry you?

Sonny: Doesn't worry me because I know why it happens. Same reason I'm claustrophobic, because my stepfather locked me in a closet. He beat the hell out of me. And it was dark in there and I couldn't breathe and I couldn't get out, so I'd get mad. And I'd have that rage and I'd hold it. I'd have to hold it inside of me, and sometimes, you know, it comes out and I don't know, it just gets the best of me, but it has nothing to do with, bipolar or chemical imbalance or whatever you want to call it.

~-~-~-~

Sonny continues to have episodes of mania, confusion, and incidents where he can't remember what happened. He threatens Alexis, but can't remember what he said. He starts working on the garden in the middle of the night. His rages are more frequent. He never wants to go to "the other side," again. He says, "I have been to hell, and I'm working my way up and trying to focus on the things that are important in my life, like my boys and Kristina, you know? They deserve the best father I can be." He gets support from family and friends.

Sonny and Emily break up. Sonny sincerely believes that he has only hurt Emily and that she deserves better. They part as friends.

"I wouldn't be getting better if it wasn't for you. You shown me that there's no shame in getting professional help. You've changed my life for the better, but all I've given you is heartache. I've been thinking about this for a while. I just didn't have the guts to face it because you were so good to me. I loved being with you, and when you came to me, when you turned to me and you depended on me, it gave me purpose. I thought I was taking care of you, when, in fact, you were fixing me. And maybe I was just sensing that, you know, another breakdown was coming, and if you were with me, because of your kindness and who you are, that it wouldn't happen. You never gave up on me. You showed me that I could get help. And, in actuality, you might've saved my life. But I gave nothing in return."

Sonny: You know how many times Jason begged me to let him kill you, starting when you chained Carly to a wall when she was pregnant with our son because you wanted to steal our baby, up until you slept with Sam? And you know why I let you live? I'll tell you why I let you live. One reason, and one reason only. Because you're my mother's son. Now Jason is dead. He was my real brother, and you had him killed. That takes all bets off the table.

Ric: You're not going to do it, Sonny. There's no way you can kill me. (Gun clicks)

Sonny: I guess fate's not going to let you die tonight.
